The Role of Metabolism in Diastolic Dysfunction & Microvascular Rarefaction

One major yet under-recognized driver of diastolic dysfunction is mitochondrial dysfunction. The heart, being the most mitochondria-rich organ, depends on these energy-producing structures for its constant contractions and relaxations. When mitochondria weaken—due to aging, illness, or metabolic stress—the heart struggles to relax properly despite having normal pumping strength. Alongside this, chronic inflammation from insulin resistance, obesity, or toxin exposure leads to myocardial fibrosis, where the heart tissue stiffens due to excess collagen, reducing its ability to fill efficiently between beats.
